Cone of Protection: Why E-Collars Are Non-Negotiable After Surgery

Ivy, a 3-year-old Poodle mix, was rushed to IVS with Ovariohysterectomy (OHE) dehiscence, a breakdown of the surgical wound following a spay procedure. Wound breakdown can range from minor surface skin separation to a complete opening of the abdominal wall, which leaves internal organs exposed.
The day before arriving at IVS, Ivy underwent a routine spay. She was not wearing an Elizabethan collar (often called an E-collar or "cone of shame") at home and was seen licking & chewing her incision. Later that night, Ivy jumped off a bed, causing her surgical incision to open and allowing a portion of her intestines to slip outside her body. She arrived at IVS roughly one hour after the incident.
Potential Complications
When abdominal organs are exposed to the outside environment, they can suffer severe damage and dryness. Bacterial exposure in the abdominal cavity can quickly lead to peritonitis (a widespread, dangerous infection of the abdominal lining) and sepsis (a life-threatening blood infection). Pets may also instinctively chew or lick at their exposed tissues, which can cause further injury and require extensive surgery to repair. Without immediate emergency intervention, these complications can be fatal.
Initial Care & Pre-Surgery Stabilization
Upon arrival, Ivy received immediate pain medication (methadone) to stabilize her. The emergency team evaluated her condition and tried to gently reposition her exposed organs, but the incision opening was too small to safely slide them back inside.
To protect her organs, the team flushed her intestines with warm saline and wrapped them in moist surgical sponges to prevent the tissue from drying out. Intravenous catheter was placed to deliver fluids and continuous pain relief. She was pre-medicated with Dexdomitor and started FLK (Fentanyl + Lidocaine + Ketamine) CRI (Continuous Rate Infusion).
The Emergency Surgery
The surgeon carefully opened the incision further to assess Ivy's internal organs. The abdominal cavity was flushed with warm sterile saline to remove contaminants. Although a portion of her small intestines and surrounding tissues showed signs of bruising and irritation, the blood flow to her organs remained healthy and strong. Her stomach, liver, kidneys, and bladder were all normal and undamaged.
After thoroughly cleaning and flushing the entire area, the surgeon trimmed away damaged tissue from the incision edges to ensure healthy skin could heal properly. The surgeon then safely closed her abdominal wall, subcutaneous layers, and skin. Ivy remained stable throughout the procedure and recovered smoothly from anesthesia.
Post-Operative Recovery & Care
While Ivy recovered well from surgery, she initially lost her appetite. To ensure she received adequate nutrients for healing and to keep her digestive tract moving smoothly, a temporary feeding tube (nasogastric tube) was placed through her nostril into her stomach.
Once Ivy stabilized, her owner elected to try continuing her care at home to see if a familiar environment would encourage her to eat. She was discharged with clear instructions, a plan to monitor for lingering infection risks, and a prescription regimen including:
- Pain Medications: Gabapentin and Carprofen
- Antibiotics: Enrofloxacin and Amoxicillin
- Appetite Stimulant: Capromorelin
- Anxiety Relief: Trazodone
Ivy was scheduled to return in 12 to 14 days for a progress check and suture removal.
Essential Strategies to Prevent Incision Breakdown
Keep the E-Collar On: Pain and itching are natural parts of healing, which prompt pets to lick or chew at their stitches. Even if your pet isn't actively licking while you are watching, keep their E-collar on continuously for 10 to 14 days until your veterinarian clears them.
Enforce Strict Rest: Restrict your pet's intense activity completely. Keep walks brief and on leash only for bathroom breaks. Avoid all jumping, running, stairs, or rough play that could put tension on the healing incision.
Stay Ahead of Pain: Administer all prescribed pain medications exactly on schedule. Preventing pain before it starts keeps your pet comfortable and reduces their urge to bother the wound.
Monitor the Wound Daily: Inspect the incision twice a day. Contact your veterinarian immediately if you notice continuous bleeding or oozing, foul odors, significant swelling, redness, separation along the suture line, or if your pet becomes lethargic or refuses to eat.
